How is the support tailored?
At Twelve Steps Housing, our support is tailored to the unique needs and circumstances of each resident, with a focus on fostering recovery and holistic well-being.
We recognise that every individual's journey to wellness is different, and our support services are designed to be flexible, personalised, and responsive to the specific challenges and goals of our residents. Here's how we tailor our support:
Individualised Care Plans: Upon admission, each resident undergoes a comprehensive assessment to identify their strengths, needs, and goals. Based on this assessment, we develop individualised care plans that outline specific interventions, support services, and goals tailored to the resident's recovery journey.
Therapeutic Approach: Our support services are grounded in evidence-based therapeutic approaches, including the Twelve Steps model of recovery. We offer a range of therapeutic interventions, including individual counselling, group therapy, psychoeducation, and skill-building workshops, to address the physical, emotional, and spiritual dimensions of recovery.
Peer Support: We recognise the importance of peer support in the recovery process. Our community fosters a supportive environment where residents can connect with peers who share similar experiences, offering mutual encouragement, understanding, and accountability.
Life Skills Development: We provide opportunities for residents to develop essential life skills that are vital for independent living and long-term recovery. This may include budgeting, cooking, time management, communication skills, and problem-solving.
Access to Resources: We help residents access a range of resources and support services to address their practical needs, such as healthcare, education, employment, housing, legal assistance, and financial stability.
Continuum of Care: Our support extends beyond the residential setting. We offer a continuum of care, with step-down services, aftercare support, and connections to community resources to support residents as they transition to independent living.
Trauma-Informed Care: We recognize the prevalence of trauma among individuals with addiction and mental health challenges. Our support services are delivered in a trauma-informed manner, emphasising safety, trustworthiness, choice, collaboration, and empowerment.
Culturally Competent Care: We respect and honor the cultural diversity of our residents and provide culturally competent care that is sensitive to their unique backgrounds, beliefs, and values.
By tailoring our support services to the individual needs of our residents and addressing their physical, emotional, social, and spiritual well-being, we strive to empower them to achieve lasting recovery, personal growth, and overall wellness.
